Working Papers

Download working papers using the link below. Please read the directions carefully. Incomplete Working Papers will NOT be processed.

Working Papers MUST have the following sections completed:

Section A → Minor’s personal information including SSN and parent signature

Section B → Employment information including employer signature

Section C → Physician’s Certification

Section F → Signature of Minor

New Jersey Working Papers (A300 Employment certification form)

Completed working papers will be processed Monday through Friday from 7:00 a.m. to 2:30 p.m.Photocopies will not be accepted. If the document is not legible, it CAN NOT be processed.
